Once you’ve opened your Mainstar Trust account (see » below) and have selected an investment, email the following required documents to purchases@mainstartrust.com or contact us at 800-521-9897:

• Completed Mainstar Trust purchase authorization form
• Completed Mutual Fund Application - Non-IRA version
  • Use the non-IRA application to ensure Mainstar completes any applicable tax filings and not the mutual fund company.
