• Completed Mainstar Trust purchase authorization form
• Copy of executed trade confirmation including:
  • The financial institution placing the trade**
  • Price the trade should be executed at
  • Dollar or share amount of purchase
  • CUSIP number

**Any applicable broker’s fees will be included in the purchase cost.
