Once you’ve opened your Mainstar Trust account (see » below) and selected an annuity, email the following required documents to purchases@mainstartrust.com or contact us at 800-521-9897:

• Completed Mainstar Trust purchase authorization form
  Completed Annuity Application – Non Qualified version
  • Use the non-qualified application to ensure Mainstar completes any applicable tax filings and not the insurance company.
